Cunard Line Ltd. is a cruise line that represents luxury and class providing four to five star cruise ship options. Their overall target market is mature adults with mid to very high income-levels. A lot of the marketing efforts put forth by Cunard were going towards tactical efforts and the promotion of individual ships as opposed to the promotion of the entire brand. Although most of the ads portrayed the same look and had an individually tailored paragraph and/or photo. Set backs caused by the worldwide recession as well as the Iraq War has caused Cunard the dilemma of risking their prestigious company image to provide consumers with larger discounts or develop other marketing strategies that will seek to maintain their 90% occupancy rate without lowering their high quality position. It created more pressure for the creation of ââ¬Å"sales orientedâ⬠marketing, which would jeopardize their high-class image. With competitors such as Carnival who are able to provide large discounts, it makes it difficult for Cunard to avoid using the same approach in order to compete. Segmentation Strategy Cunard seems to have taken a Concentrated Marketing approach to segmentation. Generally, they focus on one target market ââ¬â the upper class. Although this can be seen as a disadvantage in an economy experiencing a recession, by segmenting based on this niche income and lifestyle, Cunard is able to tailor their services specifically to them rather than targeting more than one market and not providing the best service possible. Not only do their advertisements and brand perception demonstrate this segmentation but they make efforts to communicate personally with their clients. This is demonstrated through through letters that they send with personalized marketing messages, tailored specifically to the needs of their customers. Price/Quality Relationship The concept of price/quality relationship is heavily integrated into the target market that Cunard has chosen. Along with their brand image, the high price of their services gives off a perceived high quality. Although services are generally intangible, services such as providing reservations for an entire mealtime in their five-star cruise ship is merely one example of how Cunard tries to maintain this price/quality relationship. With the price that consumers pay, they come to expect the best service for their dollar. Generally those who pay these high amounts believe that money is no object as long as they receive a great quality product or service. Satisfaction would mean that they reached or exceeded the expectations consumers had of the service. Cunard tries not to portray prices in their advertising or have constantly advertised sales in order to keep the price/quality relationship high and retain customers who respect this aspect of their company. Alternative Strategies There are multiple strategies that Cunard may take in order to maintain their high-class brand image while bringing in enough consumers from their target market. One of these alternatives is to create relationships with big companies and provide them with a vacation package deal. An example of this is creating a relationship with a large insurance company who is then able to create a contest for self-employed insurance advisors. As self-employed individuals they have the potential of gaining high incomes and through a contest they will be motivated to meet certain productivity requirements. Only those who qualify will be able to go on the trip and will therefore meet the high standards of the Cunard brand. This method would probably be more effective for the four star ships who cater less to those at or near the retired age and target a little bit of a lower income than the five star ships. Another alternative strategy would be to create a loyalty program for existing Cunard customers. This method can be used for customers of the four or five star ships. This loyalty program would provide occasional direct offers to customers who already have experience with Cunard and therefore will not view the deals as a lowering of standards. An example of a deal that can be provided is a selling package that is good for a year or two. This package would outline a discounted price for a set amount of people to travel on a certain cruise ship. That way, there is enough time for a customer to realistically plan out a trip while providing an incentive for them and encourage more action as opposed to a regular priced ad. Lastly, an alternative that can be taken is to build relationships with various large companies with employees who make an income in the target range and make offers to company executives. This method would not downplay the company image because Cunard would be communicating directly with high-income executives and furthermore will not be making any public sales or advertisements. This method would create new potential customers who are directly in the target market of choice using the method of relationship building. This may also result in additional customer retention. This method can be used for both the four and five star ships. In order to attract the target market of the five star ships, Cunard can make an arrangement with a company to offer a deal with their luxury ships as part of a retirement plan. Recommendations As a general recommendation, Cunard should focus on brand loyalty and relationships. Marketing efforts should go towards the advertising of the overall brand rather than individual ships while the rest of the companyââ¬â¢s efforts should go towards relationship building. These relationships will then in turn target certain customers to certain ships through Cunardââ¬â¢s arrangements with companies. Considering the situation regarding the recession and war, Cunard should focus heavily on the strategy of building relationships with companies who can create vacation contest packages for their employees. During the time of a recession, anyone would appreciate an incentive to make more money and the opportunity to gain something from it. By winning something so prestigious, consumers gain confidence as well as social benefits, which can initially be their motivation. If it is a large company, there is potential for many qualifiers who can turn into loyal customers whether it be through the continuation of the contest yearly, or by their own personal desire to continue their relationship with Cunard. These contests will not jeopardize the high-quality image of the company because through negotiation, Cunard can insure that the qualification requirements are at a level where when achieved, it will be viewed as prestigious. If this contest continues yearly, Cunard can maintain their customer value, satisfaction and trust and therefore have customer retention. This will result in less price sensitivity, positive word of mouth, returning customers and a long lasting relationship. If Cunard continues to use marketing to market their entire company while bringing their services directly to their desired market, they can ensure a 90% occupancy and the continuation of a high-class cruise ship operation.

The biographical drama Faith like Potatoes has very deep meaning and inspirational power. Also, many Christian films not only lack a compelling story line and too easily slip into sentimentalism but the story of Angus Buchan is told beautifully in this visual art form and the message of prayer and faith. To begin with, I would say that the drama reveals much of what nowadays people face to- the crisis of faith. Angus is like portrayal of nowadays humanity. Even if he lives in South Africa where it seems that time should be less rapid and a person would have time for himself, Angus faces to the same problem as people living in a fast moving world. I am not sure whether I could call it a problem or obstacle that Angus and many other people face to- a frustrating mode of life. As a result it causes anger, loss of self-esteem and self-confidence, and stress and depression. My point is that, ordinary people and the character in a movie face to the problem which is the lack of time, causing frustration and leading to abundance of oneself inner voice. The result is that people start looking for help which is a true relationship with God. Still, it is said that when people face to personal crises, they try to find help through faith and then changes occur, which usually are spiritual growth. The same happened with the character in the movie. As a result, changes started with Angus personality, attitude toward his family, faith, and work, too. Here I could say that work and faith are very closely related and important. Work is essential for Angus and his family. Farming is the only way that helps tem to live on. Later in the movie, I could say that farming become as a mean to express faith and as a way to glorify God. At the point, theological argument occurs whether God created man to glorify him? The fact that God cannot be seen does not mean He cannot be known. Some men have chosen not to know God as they have denied Him by claiming that the world evolved randomly and naturally. But, if one were to look at the order of things as being held constant by the Creator, who created all things, one can begin to see the God who was and is powerful enough to create such a beautiful place even though He cannot be seen directly. In short, what makes Angus story such an inspirational is that it makes to consider ones own life. It inspires to be a better person, to believe more, and to have unwavering faith in the power of God. Moreover, the movie has many theological points to be brought up and discussed.

Congress has exercised this delegated power in many cases. The nature and basic guidelines of Congressââ¬â¢ power over commerce is first laid out in the case of Gibbons v. Ogden. In addition, the case United States v. Lopez is a prime example of Congressââ¬â¢ ability to carry out the Commerce Clause to the furthest extent. Lastly, the case National Labor Relations Board v. Jones & Laughlin Steel Corporation brings to light the Wagner Act of 1935. T hrough a review of these three cases, it can be concluded that there are no real limitations on Congress when regulating commerce. The Constitution of the United States explicates the enumerated powers that the people have granted to their public administration. A narrow interpretation of the Constitution would mean denying the government the powers granted to them to keep order, equality, and fairness. The action of bringing it does directly violate the Gun Free School Zone Act, which the Federal Government made a federal offense. It is a poem about how the poet had fallen in love but it turned out it was unrequited. Whereas ââ¬Å"Songâ⬠by W.H.Auden written in the 20th century, is a poem about how someone has been in love but then lost them to death. They are both quite similar in the fact that they are both about loving someone but not being able to have them. However they are different because ââ¬Å"Songâ⬠is about two people having been in love and then losing it, rather than ââ¬Å"First Loveâ⬠in which the love is unrequited, and not being fulfilled. In the poem ââ¬Å"First Loveâ⬠by John Clare the poet writes about what seems to be a very overwhelming feeling. The poem is written in three stanzas and in each one the feelings develop. It has a rhyme structure of AB,AB, CD, CD etc. The first stanza has eight syllables in each line and the other two have a pattern of 8,6,8,6,8,6,8,6. I think it may be written like this because in the first stanza the feeling are simpler and then they get more complex as the poem progresses- like the syllable patterns. In stanza one the crush begins. He sees her and is suddenly struck by her beauty- ââ¬Å"Her face it bloomed like a sweet flower.â⬠This simile is saying that her face opened up and revealed something beautiful just like a flower does. It brings the image of spring to mind because flowers blooming are associated with it. The Role of Women in Sir Gawain and The Green Knight Sir Gawain and The Green Knight is an example of medieval misogyny. Throughout Medieval literature, specifically Arthurian legends like Sir Gawain and the Green Knight, the female characters, Guinevere, the Lady, and Morgan leFay are not portrayed as individuals but social constructs of what a woman should be. Guinevere plays a passive woman, a mere token of Arthur. The Lady is also a tool, but has an added role of temptress and adulteress. Morgan leFay is the ultimate conniving, manipulating, woman. While the three women in this legend have a much more active role than in earlier texts, this role is not a positive one; they are not individuals but are symbols of how men of this time perceive women as passive tokens, adulteresses, and manipulators. Guinevere from the very beginning of the legend is portrayed as a passive, typical lady of the court. In stanza four, the author describes Guinevere almost as a trophy or ornament of the court: "Queen Guinevere very gaily was gathered among them/....The prettiest lady that one may describe/She gleamed there with eyes of grey/To have seen one fairer to the sight/That no one could truly say" (74-84). Guinevere does not take an active role in the court. She does not have speaking role and basically just sits among the knights of the Round Table. Her passivity and silence could be the result of medieval anti-feminism. According to Bloch in medieval times what a woman wants is to speak. Medieval authors such as Andreas Capellanus, the supposed author of The Art of Courtly Love writes, "Furthermore, not only is every woman by nature a miser, but she is also envious, and a slanderer of other women......fickle in her speech,....a li... ... because of her beauty. Genting Plantation Berhad
Genting Plantations Berhad (ââ¬Å"Genting Plantationsâ⬠), formerly known as Asiatic Development Berhad, is one of the fastest growing plantation companies listed on the Main Board of Bursa Malaysia (formerly known as Kuala Lumpur Stock Exchange). Incorporated in Malaysia as a private limited company on 29 September 1977, under the name of Asiatic Development Sdn Bhd, it became a wholly owned subsidiary of Genting Berhad (ââ¬Å"Gentingâ⬠) on 22 February 1980 to spearhead Genting's plantation business. Listed on Bursa Malaysia on 30 August 1982, Genting Plantations currently ranks amongst the top 10 listed companies in terms of market capitalisation in the plantation sector. As at 30 April 2009, Genting Plantations is a 54. 7% owned subsidiary of Genting Berhad. In April 1980, Genting Plantations commenced business through the successful acquisition of the Rubber Trust Group comprising 3 Hong Kong domiciled rubber companies which owned some 13,700 hectares of plantation land in Peninsular Malaysia. In June 1981, the Ko Rubber Companies, which owned about 10,000 hectares of plantation land in Peninsular Malaysia, were acquired by Genting Group and through a restructuring exercise in 1983, some 9,268 hectares of these plantation land and businesses were transferred to Genting Plantations. In the same year, Genting Plantations acquired a further 676 hectares of oil palm plantation and a 30 tonne per hour oil mill. These acquisitions would have now proven to be most astute decisions judging by the development potential of some of the land and the capital appreciation that has since taken place, particularly the 3,200 hectare ââ¬â Genting Kulai Besar Estate which is now being developed as the mega Genting Indahpura project in Kulai, Johor. Recognising the scarcity of land in Peninsular Malaysia, Genting Group started making inroads into Sabah. In 1985, Genting Group acquired Asiatic SDC Sdn Bhd, whose plantation business comprised some 4,000 hectares of oil palm plantation and a 20 tonnes per hour oil mill. In a subsequent rationalisation of Genting Group's plantation activities, ownership of ASDC was transferred to Genting Plantations in 1991. A quick succession of land acquisitions in Sabah ensued for Genting Plantations, starting with the takeover of Tanjung Bahagia Sdn Bhd in 1988 which owns 8,094 hectares of jungle land, followed by a joint venture arrangement with Landworthy Sdn Bhd (now known as Asiatic Tanjung Bahagia Sdn Bhd) in 1989 which added another 4,039 hectares. Between 1991 and 1996, another 5,730 hectares were acquired. With all these jungle lands fully cultivated, Genting Plantations, with its long term vision to further its plantation interest, continues to expand its land bank. Notable acquisitions include the 8,830-hectare agricultural land near the Sungai Tongod and the 5,611-hectare oil palm estate near Bukit Garam in 2001 and 2002 respectively. And in May 2004, Genting Plantations has completed yet another acquisition of some 6,753 hectares of oil palm estates together with a 60 tonnes per hour oil mill in Kinabatangan, Sabah. In June 2005, Genting Plantations embarked on its maiden foray into the plantation business overseas. This expansion of plantation activities in the Republic of Indonesia is on a joint venture basis with a respectable palm oil producer in the country, the Sepanjang Group. The Genting Plantations-Sepanjang 70:30 joint venture will develop some 76,000 hectares of agricultural land into oil palm plantation. Genting Plantations entered into another joint venture in 2008 to develop 45,000 hectares in Ketapang. Following these recent acquisitions, Genting Plantations's current land bank is slated to increase to over 186,000 hectares in Indonesia and Malaysia; an impressive 14-fold increase since 1980. The Group continues to be actively working towards adding more. At present, Genting Plantations owns 6 oil mills, one in Peninsular Malaysia, which attained the ISO 9002(1994) Quality Management Systems status in 2001 (which was subsequently updated to the ISO 9001(2000) in 2002) and the others in Sabah. Total milling capacity to date is 265 tonnes per hour. In 1994, Genting Plantations entered into a joint venture with a Sarawak state agency to establish a 30/60 tonnes per hour palm oil mill in Serian, Sarawak. This mill scored a hattrick when it was awarded the Anugerah Industri Sawit Malaysia, in recognition of its high oil extraction rate, by PORLA since its commissioning in August 1997. Whilst plantation business remains the core activity, Genting Plantations has since ventured into property development with the objectives to unlock the potential of its strategically located land bank and to reduce its dependence on single source income. The first project by Genting Plantations's property arm, Genting Property Sdn Bhd (ââ¬Å"Genting Propertyâ⬠) is the 55-hectare Genting Cheng Perdana in Central Melaka. Officially launched in October 1993, the project has turned out to be quite a success with its first phase of single- and double-storey houses completed and handed over to purchasers about 9 months ahead of schedule. Since then, 3 other phases have been completed and handed over in good time. What followed next was the launching of the 284-hectare Genting Permaipura project in late 1994. Located about 10 km from the fast-growing town of Sungai Petani, Kedah, a town poised to become the commercial/industrial hub of the northern region in Malaysia, Genting Permaipura comprises commercial and entertainment centres, bungalow lots, residential houses and shop-offices. Also included in this project is an 18-hole golf course with a 2-tier driving range which sprawls over 160 acres of prime land against the majestic backdrop of Gunung Jerai and a clubhouse, The Genting Permaipura Golf & Country Club. Further, in its push to the forefront of property development, Genting Property had embarked on another project ââ¬â the massive Genting Indahpura or ââ¬Å"Beautiful Cityâ⬠project at Kulai, Johor. Spanning over 30 years, this project located just 30 km north from the bustling and fast growing city of Johor Bahru involves the development of about 3,200 hectares of plantation land in Kulai, Johor, into a self-contained township. Genting Indahpura is conceptualised as a fully-integrated development complementing the State's plan to upgrade the existing Kulai town into a sub-regional centre complete with all modern amenities and convenience, and generously landscaped town parks and green lungs for healthy and recreational pursuits. The project's attraction lies in its excellent location which enjoys easy access from virtually every transportation route ââ¬â road linkages through the North-South Expressway and the Second Link from Singapore; air linkages through the Sultan Ismail International airport in Senai 10 minutes away; and rail links through the Federal Railway lines. Genting Plantations also has other large tracts of land with development potential. These will continuously be monitored as to the timing and the type of development most suited in relation to their locations. With low land holding cost and huge reserves of own land bank, Genting Plantations can aspire to play a bigger role in the property sector in Malaysia in the near future. Moving forward, the Group, through its wholly owned subsidiary, Asiatic Centre for Genome Technology Sdn Bhd (ââ¬Å"ACGTâ⬠), has ventured into the biotechnology industry in 2006. ACGT has set up an equally owned joint venture with Synthetic Genomics Inc ââ¬Å"Synthetic Genomicsâ⬠), a privately held company dedicated to commercializing synthetic genomic processes and naturally occurring processes for alternative energy resources. The joint venture is tasked to carry out research and development activities on the use of genomics-based techniques and tools to increase the yield and profit stream from crop plants. It will initially focus on whole genome sequencing of oil palm. Synthetic Genomics, which was founded by genome pioneer J Craig Venter, Ph. D. will use its expertise in DNA sequencing to enable SGSI-Asiatic Limited to develop a comprehensive genomic study of oil palm whilst ACGT will be the exclusive agent and sole licensee, on a worldwide basis, for commercialising the technologies developed. Management of the seas of Southeast Asia Essay
Comprehensive marine and coastal databases are essential for management of the seas of Southeast Asia. Maps of resources found in marine and coastal environments, the sources of potential damage and data to guide managers are the means by which resources can be assessed and managed to maintain marine resources. This information is get by do the oceanographic survey. There is a strong need for data for managers to manage, conserve, remediate, recover and sustain continued use of Southeast Asian coastal and marine resources. This need is met by inspired, knowledgeable and scientific use of data. Also, the importance of monitoring, mapping and management is considered. Currently there are many global databases including global datasets of a general nature. There are also several national databases that are more specific, but access to the information in many of these databases is difficult from an international perspective primarily because of the language barrier. However, there are few regional databases and the existing ones include limited sets of data and information, often collected under the umbrella of the hosting organization. It is very difficult to get a comprehensive overview of which data and information are readily available from public sources at global, regional and national levels and this makes it difficult to assess data and fill information gaps. There is also very little information providing an overview of past or on-going project activities in the region and the lessons-learned from these oceanographic survey activities. Likely reasons are the difficulties in collecting such information and the need for regular verification and updating. In addition, no organization has taken the responsibility, or has been given the mandate, to collect and coordinate information on activities under the various organizations and programmes operating in the region. Data ocenography take time and money to collect and are extremely valuable resources. They affect coastal and marine management decisions and need to be stored and shared. In order to make best use of the efforts undertaken in the Southeast Asian Seas region, increased collaboration between existing projects and programmes would be beneficial in order to increase data oceanography accessibility and address information gaps at national and regional levels. This collaboration may well start with increased information sharing and networking between regional organizations, governments, organisations,Ã universities and existing database initiatives through further developing mechanisms.

Hundreds of years ago, people who lived in South America started to produce coffee, but the earliest drink likes coffee was made by ancient Arabian. They thought it was a kind of medicine used for stomach. After the 15th century, coffee was spread to other countries such as Egypt and Ottoman Empire by Muslims who were back from Hajj, while in ancient China, Chinese people started to make tea about 3000 years ago. Because of nice environment and weather, the first part of tea plants was discovered in southwest China.After, drink tea became an important culture of Chinese people, and tea began to spread into countries near China. Coffee and tea are similar, but different in some specific area. First, coffee and tea are similar but different in population. People from all around the world like them. For example, British people used to drink a cup of tea with some snacks when they have a rest in the afternoon. Weste rn people enjoy drinking black tea, and some of them mix it with milk to make milky tea. Likewise, most people like drinking coffee, for it often tastes good.In China, more and more people began to drink coffee, and some of whom want to taste better coffee like blue mountain coffee. However, although these two drinks are both popular around the world, coffee is still drunk most by western people, while the most quantity of tea is used in East Asia. People lived in China, Japan, and Korea regarded tea as the main drink. Second, coffee and tea are similar but different in function. Both coffee and tea can refresh people because there is a kind of chemical matter, which is caffeine in both of them.This chemical matter can be used for nerves to make a person feel awake and lively. Also, some experts said that black coffee and green tea can help a person lose weight. As such, Juvenile detention officers are responsible for making sure that inmates arrive on time to appointments within their facility and outside of it. However, in some unfortunate cases, not everyone has the chance to grow up with a traditional family. The idea is, oneââ¬â¢s family should be the biggest and the most important supporters in their lives. We define our familes different, whether it is a mother, a father, brother, sister, grandparent or even your best friendââ¬â¢s family. The point is having the sense of strength in oneââ¬â¢s home is important to oneââ¬â¢s success or failure. Strengthening families can be accomplished by offering help to those that need it through information and education. Which leads to my second point is education. Juveniles feel lost as do most of us that donââ¬â¢t know what to do or which direction to head to. Through education we get an idea of where we would like to go in our lives. Some juveniles do not have access or do not see the importance of education until there is support, programs, and resources available to them. We promote education through the media and all sorts of things, but are we really making it available for everyone? The Clostridium Difficile Infections Biology Essay The organism known as Clostridium difficile is a gram-positive bacillus bacteria which has the ability to form spores, as well as produce a number of toxins. In addition , the organism has well known epidemiology, with certain patient attributes, exposure to high-risk environment, medical conditions and various medications contributing to an increased risk of both the asymptomatic Clostridium difficile colonization (CDC) or the symptomatic and sometimes deadly Clostridium difficile infection (CDI). CLOSTRIDIUM DIFFICILE Pathogenesis Clostridium difficile (C. difficile) are gram positive, spore-forming bacillus bacteria which, as an opportunistic pathogen, inhabit the anaerobic conditions of the human gastrointestinal system. The reason that it can be so detrimental in the case of an infection is that along with a number of other virulence factors, it produces two toxins, known as toxin A and toxin B (CDC, 2012). In patients who display either a colonization or infection, the normal gastrointestinal flora is depleted due to a number of extenuating risk factors. Provided with these circumstances, the C. difficile bacteria are able to flourish and overrun the patients bowel. The major agressins (Borriello, 1998) of C. difficile are u ndoubtedly toxins A and B, however, there are a number of other virulence factors possessed by the organism which contribute to its potential to cause harm. According to Borriello, C.difficile is influenced by its ability to adhere the intestinal wall, which may be caused by the organisms intrinsic slight positive net charge. This attracts to the negatively charged host cells [and] may contribute to gut colonization (Borriello, 1998). Both toxins A and B are cytotoxic to a very large number of different cell types, both cause increased vascular permeability, and both cause haemorrhage (Borriello, 1998). In addition, toxin A appears to cause fluid accumulation, whereas toxin B does not (Borriello, 1998). Clinical Features Immediate clinical symptoms of C. difficile can include fever, loss of appetite, nausea, abdominal pain and tenderness (PHAC, 2011) as well as watery diarrhea. The diarrhea is a by-product of the toxins produced by the multiplying bacteria as they invade the mucosa of the intestines. This causes profuse inflammatory diarrhea secondary to destruction of the lining of the colon (4). In more severe cases, it can cause pseudomembranous colitis, bowel perforation, sepsis, and even death (PHAC, 2011). Diagnostic Methods There are currently several reliable, widely-used laboratory tests which are used in the diagnosis of C. difficile colonization and infection. Microbiological stool culture is the most sensitive test available (CDC, 2012) and is considered the confirmatory test, but it also carries the highest incidence of false-positives. This occurs when the patient is infected with a non-toxigenic strain of C. difficile. PCR assays have been developed for the gene which encodes for toxin B. In addition, antigen detection by either latex agglutination or immunochromographic assays (CDC, 2012) provides a fast way to detect the presence of Clostridium difficile. Again, it is non-specific for toxigenicity. Toxin testing tests for specificity to toxin B, while enzyme immunoassays can detect either toxin (CDC, 2012). As studied by Kinson in 2009, additional testing for various markers is also being investigated as a means of detecting infections. Examples of this include fecal lactoferrin, a marker for intestinal inflammation (Kinson, 2009) as well as glutamate dehydrogenase (GDH), which is C. difficile-specific [à ¢Ã¢â ¬Ã ¦] however GDH positivity is independent of toxigenicity in strains of C. difficile (Kinson, 2009). Although its presence does confirm Clostridium difficile is present in the patient, it does not confirm that the strain present in this patient is toxigenic. Therapeutics According to the Public Health Agency of Canada, mild cases of CDI can resolve with only supportive treatment such as intravenous fluids to combat dehydration (PHAC, 2011). Additionally, the Centre for Disease Control states that up to 20% of cases will resolve within two to three days of discontinuing the antibiotic to which the patient was previously exposed (CDC, 2012). In more severe cases, the infection can usually be treated with an appropriate course (about 10 days) of antibiotics, including metronidazole, vancomycin (administered orally), or recently approved fidaxomicin (Aylin, 2011). If the bacteria have severely damaged sections of the bowel, it may have to be removed surgically as well (Louie, 2004). EPIDEMIOLOGY Risk Factors for Infection The incidence of infection by Clostridium difficile is affected by a number of risk factors, which is depicted in Figure 1 (Owens, 2008). Being hospitalized greatly increases the chances of becoming infected with C. difficile. These bacteria are shed in the feces, and are usually transmitted between patients either by healthcare workers, or by surfaces or equipment not being fully sanitized between patients (Louie, 2004). However, there has been an increasing trend of community-acquired infections as well. In a study performed at Harvard Medical School, it was found that community-acquired Clostridium difficile infection may account for more than a third of Clostridium difficile-associated diarrhea overall (Leffler, 2012). In addition, the use of medications such as antibiotics, particularly fluoroquinolones (Bourgault, 2011), as well as proton pump inhibitors (used to supress production of gastric acid in gastrointestinal conditions) have been shown to increase the risk of a Clostri dium difficile infection. In a study by Haider et al, it was shown that while the use of proton pump inhibitors appears to lead to an elevated risk of developing severe CDI (Haider, 2012), another widely used type of gastric acid suppressant medication known as histamine 2 receptor antagonist (H2RA) actually appears to decrease the risk of an infection (Haider, 2012). Gastrointestinal surgery is also a known risk factor for severe infection with Clostridium difficile (Louie, 2004). According to Public Health Ontario, infections are more likely to be considered severe in an elderly or immunocompromised patient (OAHPP, 2011). However, it has been shown that the presence of multiple medical conditions, or co-morbidity, is actually a better predictor then age as to the outcome of the infection. Severe CDI occurs more frequently with advancing age. However, age, per se, has no effect on mortality (Dharmarajan, 2000). IMPLICATIONS IN PUBLIC HEALTH Resource Allocation It has been shown that both the financial implications, as well as the allocation of resources within the health care system produced by Clostridium difficile-associated disease (CDAD) are quite significant. Public Health Ontario stated at the time of their study in 2010, that the cost of CDI readmissions alone is estimated to be a minimum of CAD $128,200 per year per hospital (OAHPP, 2011). A more extensive look in to the associative costs was completed in 2008 at Washington Universitys School of Medicine. Dubberke studied a population of CDAD patients and proposed that a cost of $2454 was attributed to each case of CDAD, with that cost increasing to $5042 per patient if their stay exceeded 180 days of hospitalization (Dubberke, 2008). According to the study conducted in by Dr. Forster et al (2011), an infection with C. difficile extends the patients hospital stay from an average of 8 days to an average of 34 days (Forster, 2011). This not only increases the burden on health care wo rkers, but also utilizes time and supplies which are quite preventable. CONCLUSION

One Flew Over The Cuckoo's Nest ââ¬â The Movie à à à à à à à à à à à The movie, One Flew Over the Cuckoo's Nest, tells the story of McMurphy, a convict, who is sent to a mental institution because he believes he is insane.à In actuality McMurphy, is sane when he comes to the mental ward, he only wants to get out of the work that jail time entails.à It is believed that his stay in the mental ward is what drives the man insane.à While in the mental ward, he interacts with the patients of his ward and ends up changing their worlds completely.à When two different societies are combined, they undoubtedly will change one another.à This is the case when McMurphy coming from the "real" world, a society where a person can do what he pleases, is associated with the mental ward patients, whose lives are completely controlled by their nurses and their routines.à McMurphy and the patients have a significant effect on each other. à à à à à à à à à à à The mental ward and the world that McMurphy comes from are completely different.à The mental ward is completely based on rules.à The patients' lives are based on the routine that their nurse, Nurse Ratched, has established for them.à Nurse Ratched believes that the rules she sets for the patients are in their best interest or getting better.à The nurses have entire control over the patients.à They are locked into their beds every night, get up at the same time, they eat at the same time, and they watch tv at the same time every day.à The patients follow Nurse Ratched's rule without ever questioning them.à Basically, they have no minds of their own.à McMurphy comes from a society almost opposite of the mental ward.à He has lived his whole life doing what he wants.à He has never had a nurse hovering over him telling him what he has to do at all time.à Being in prison shows that McMurphy has a hard time living by the rules.à So living by strict rules of the mental ward is going to be even harder for him. à à à à à à à à à à à Living in the mental ward is very hard for McMurphy at first.à The patients and McMurphy cannot understand one another so socializing with them is hard for him.à When he begins to interact with them, he has a profound effect on the patients of the mental ward.
